Berg Balance Scale (BBS)

A 14-item scale that assesses static balance and fall risk in older adults and individuals with balance impairments.

  • Commonly used for neurological patients
  • Assesses fall risk on a 0-56 point scale
  • Takes approximately 15-20 minutes to administer
  • Validated for use with older adults and stroke patients

Tinetti Balance Test (POMA)

Assessment of balance and gait to determine fall risk, also known as the Performance Oriented Mobility Assessment.

  • Includes separate balance and gait sections
  • Total score of 28 points possible
  • Scores under 19 indicate high fall risk
  • Takes approximately 10-15 minutes to complete

How to Use These Resources

These assessment tools are provided for clinical use by qualified healthcare professionals. Here's how to use them:

Printing Instructions

  1. Click on the assessment tool link to open it in a new tab
  2. Use the "Print This Page" button at the top of the page
  3. Adjust your printer settings as needed (portrait orientation recommended)
  4. Print directly or choose "Save as PDF" option in your browser's print dialog

Note: The forms are designed to print clearly in black and white with proper spacing for clinical use.
